WelbeHealth’s PACE programs serve medically complex and underserved seniors, helping them live independently at home through fully integrated medical, behavioral, and social care. Interdisciplinary teams provide wrap-around support, including medical services, personal care, transportation, meals, and social activities. WelbeHealth operates eleven centers across California and is continuing to expand, with new locations opening this year in Elk Grove and San Bernardino.

A board-certified internist with fellowship training in infectious disease from UC Irvine, Dr. Hirota brings more than two decades of experience leading value-based care models for complex populations. Before joining WelbeHealth, he served in leadership roles at Private Health Management and Landmark Health, where he helped scale interdisciplinary care models across multiple markets.

Dr. Hirota’s mandate is to continue driving key priorities for the organization’s care model, including:

Strengthening physician-led excellence in clinical outcomes, including hospital readmissions, chronic disease management, and vaccination rates
Expanding access to integrated behavioral health and palliative care services
Developing and mentoring leadership across WelbeHealth’s Medical Practice
